At Hometown Family Dental you are more than just our patients, you are our family. We are excited to offer membership plans to our uninsured patients to make care affordable.

  • Our membership plan is not insurance and is not a qualified health plan under the Affordable Care Act.

  • Our membership plan includes a 30-day money back guarantee.  You can receive a full refund up to 30 days after you purchase if no benefits of the plan have been used.  Refunds after 30 days will be at the discretion of our practice.

  • Payment for any treatment(s) not included in the membership plan is due at the time of service.

  • The membership plan benefits may not be combined with insurance, other offers or discounts.

  • Our membership plan does not cover the following procedures completed at our practice:

    • Orthodontics such as comprehensive orthodontic treatment

    • Appliance Therapy (CDT Code D8000 – D8999)

  • Our membership plan does not cover procedures completed by specialists outside our practice

  • All benefits for the plans must be used within 365 days. There is no roll over to next year(s) of fees unused or benefits of plan.

  • You cannot transfer benefits from yourself to other people or another person.

  • Dr. Smith will help you decide which plan is right for you and she has the final word regarding which plan you can subscribe to.

  • For Plan 4 members, the initial cleaning appointment(s) for unhealthy gums (such as “scaling,” “deep scaling,” “root planning,” “deep cleaning,” or “periodontal surgery,” etc.) may NOT be included in the year membership fee. You will receive a 30% break/discount on those procedures, however. “Periodontal Maintenance,” or the cleaning you have three times per year is included under Plan Two.
